ControlFreak is a Microsoft Excel template (running under Windows), which means that you can enter your data in the same way as you would do in any regular spreadsheet. The data can be entered manually or by querying a database. Here is how it may look:

After you have entered the data into the spreadsheet you will open the Main Form to direct how the control charting should be done (Click on the image to enlarge it):
